The Centers for Disease Control and Prevention (CDC) is the primary federal funder of state and local public health infrastructure, distributing cooperative agreements through programs like the Public Health Emergency Preparedness (PHEP) cooperative agreement, the Epidemiology and Laboratory Capacity (ELC) program, and HIV/STI prevention cooperative agreements. HRSA — the Health Resources and Services Administration — funds the primary care workforce through its Health Center Program (FQHCs), rural health programs, and the National Health Service Corps. State and local health departments are the most common CDC primary award recipients; universities, FQHCs, and nonprofits access public health funding both as direct CDC awardees and as subrecipients through state health department pass-through grants.
