HHS-HRSA — Health Resources & Services Administration

FQHCs are community-based health care providers that receive HRSA Section 330 Health Center Program funding. To become an FQHC, an organization must apply during a HRSA funding opportunity period, demonstrate service to a medically underserved area or population, offer a sliding fee scale, operate under community board governance (majority patients), and provide comprehensive primary health care services. The application process is competitive and multi-year.

HRSA rural health grants are available to public and nonprofit organizations located in or serving rural areas, including rural hospitals, health departments, tribal health organizations, and rural health networks. Many programs require an existing health care infrastructure or partnership with rural providers.
